Fellow Assessment Tools: Surgical Log and Milestones

 
The Fellow Assessment: Surgical Log and Milestones form should be submitted bi-annually by Fellowship Training Program Directors. Forms should be submitted in January and July regardless of the fellow's start date. The assessment should include a copy of the fellows surgical log report, an operative milestone assessment, and a non-operative milestone assessment. For the milestones, if you are having more than 1 faculty member assessing fellow(s) please average the assessment and send only 1 form.  

For the surgical log approval, your fellow is responsible for sending you their surgical log report from the Academic Universe, which you will then upload when you complete the form. 

Note, you must sign in to your ASTS Member Portal account to access the submission form. If you are submitting the form for more than one fellow, you will need to revisit the milestones link after submitting the first form and go to “Click here to begin a new submission.”

Program Accreditation Information

 
The Transplant Accreditation & Certification Council (TACC) has established guidelines and standards for programs to earn and maintain Fellowship Training Program Accreditation. It is the responsibility of all programs and fellows to review the document and be familiar with the requirements.  

Fellowship Training Director Job Description

 
The Fellowship Training Director job description was developed to offer guidance to Accredited Training Programs and Fellowship Training Directors to assist in understanding the responsibilities associated with Abdominal Transplant Surgery Fellowships. 

Transplant Accreditation & Certification Council 

The Transplant Accreditation & Certification Council (TACC) was established by ASTS in January 2017 to oversee accreditation functions and develop a pathway for fellow certification.  

Match Violations

The Transplant Accreditation & Certification Council (TACC) is responsible for protecting the integrity of the SF Match process by ensuring that all match participants and Fellowship Training Programs follow the rules and policies established by SF Match and TACC.
